Each of the programs below have the Position Statement or Guideline and Authorization form with them. The Position Statement or Guideline outline the requirements for the specific program. Please review the Position Statement or Guideline carefully before completing the Authorization Form for submission to the CVO office. Any clarification may be obtained by contacting the CVO office.

The Authorization Form must be completed and submitted to the CVO office for approval at least 2 weeks prior to the day of the program.
